Job duties include:

• Assist the client in completing the program’s welcome packet
• Responsible for transporting clients from their homes to the program and back
• Assist clients’ assessment meetings with the psychiatrist
• Track and schedule individual client’s monthly evaluation meeting with the psychiatrist
• Assist clients with their lunch menu
• Keep clients’ daily attendance record
• Keep monthly client admission and discharge records
• Audit client’s chart for missing signatures or documents

CNA preferred. A vehicle will be provided by the hospital for transportation.
